3) If the person going to the Business Office is not the subscriber on record, then you must also bring a SIGNED Letter of Authority from the subscriber on record.

The subscriber on record is the name indicated in your phone bill.

4) A myDSL subscriber may only avail of this promo once the DSL connection has already been installed. Guild Wars is best played on a DSL connection so PLDT will only allow you to get a copy of the game once you have a working DSL connection.
